What to expect during the commercial driver’s physical examination?

The doctor will talk to you and obtain a brief history.  This is followed by a physical examination.  A vision test will be done so don’t forget to bring your contact lenses or eye glasses.  Also, a urine sample will be tested for blood, protein, and sugar.

“We supply the Exam Form, Medical Certificate, Laminated Wallet Card, and a Folder to hold your documents”.

Items you NEED to Bring to your DOT Physical Exam:

  • State photo identification, driver’s license is recommended
  • A full bladder, we need a urine specimen (or you’ll be staying until you can provide one)
  • If you are re-certifying, you need to bring your current/expired medical card
  • If you wear glasses, contacts or use hearing aids, bring them
  • A list of all procedures, medications or healthcare related documents for the past 5 years
  • Bring any waivers or exemptions for any medical reasons
  • Bring medical releases, information or documentation for any illnesses, conditions or injuries you have been treated for in your past or present
  • If you use a CPAP – you must bring a log of usage of 1 year and provide the last time you have been examined by a doctor, for its use

We supply all the necessary forms.
